It is hard to determine precisely why the nurse did not follow safety protocols without further details; however, some probable reasons could be lack of awareness, distraction, time constraint, or even negligence. In nursing practice, particularly in the context of blood transfusions, accepted protocols include double-verifying the patient's identity with two identifiers, such as name and date of birth, labeling test tubes correctly at the bedside, and meticulously documenting every step. NMBA's Standard 5 focuses on proper documentation, while ACSQHC Standard 2 focuses on involving patients and families in care decision-making. Ideally, a second nurse ought to verify all the labels on the blood samples and match them with the correct patient paperwork prior to processing. In this instance, the nurse omitted these safety steps, either voluntarily or involuntarily, leading to a fatal error. As a student nurse, it is important to learn these guidelines by heart and instill the habit of meticulously adhering to them, however tedious the process may appear.
