C. Based on Tony’s current lifestyle, following modifiable risk factors can be done which are diet and salt intake due to which risk of high blood pressure and hypertension gets increased. This risk factors can be prevented and controlled with the help of nursing intervention which are:

This nursing intervention will include modification of diet plan and workout which is also provided on the VUWS site, his diet must be planned in such a way that it consist lesser amount of salt and higher amount of fiber in it as well as it must be fresh and cooked in minimal amount of oil and he must be convinced to follow the same. His consumption of junk food such as deep fries drinks and soft drinks and eight cups of coffee a day should be restricted. Diet plan must also involve detail of amount of fluid which must be consumed in a day. Weekly workout schedule can be planned, in which different workouts plans can be included such as cardio exercise, walk, yoga or cycling, alternatively. First step in intervention must be, to inform the patient regarding his health issue and the next step must be to convince patient to change and implement the above changes in his daily life (Lacoste, et al. 2010).
